Licensed Practical Nurse LPN Job Summary

As the Homecare Licensed Practical Nurse LPN you will perform selected nursing care for Patients in Care of Sick andor Hospice Programs as delegated by the PCN whom works in a nursing team relationship under general supervision of the Clinical Supervisor.

Licensed Practical Nurse LPN Job Responsibilities

• Collaborates with PCN to review Plan of Care and provide nursing care to patients in their homes on a family centered basis while recording maintaining complete accurate patient record on Portable Electronic Device andor via paper record.
• Makes patient assessment reports and documents appropriately as delegated by the registered nurse within their scope of practice.
• Works under direction of PCN in orientating and delegating responsibility to the Home Health Aides within Regulatory Guidelines as per agency policy.

Licensed Practical Nurse LPN Education and Experience Requirements

• LPN Degree required.
• Current CT license required.
• Minimum of four 4 years of nursing practice two 2 years of which must be recent Med-Surg acute care or critical care experience as a Licensed Practical Nurse preferred.
• Valid Connecticut Drivers License and proof of current insurance coverage required
