Job Summary We are seeking a Licensed Practical Nurse to join our team. In this role, you will be caring for Hospice patients in their homes. You are a Licensed Practical Nurse (LPN) with great attention to detail and a big heart. You take pride in caring for patients following personalized care plans for patients. You are able to assess, observe and communicate effectively regarding your patients care.
Responsibilities
Coordinate with other colleagues to assess, plan, implement, or evaluate patient care plans
Administer medications and monitor patients for side effects or reactions
Document any changes in patient conditions and report on changes
Maintain accurate records of patient care and discuss observations with the supervisor
Qualifications
Currently licensed Licensed Practical Nurse (LPN) in state of Ohio
Previous experience as a Licensed Practical Nurse is preferred
Hospice experience preferred but not required
Commitment to excellent care and customer service
Excellent interpersonal and client care skills
Valid driver’s license
First aid/CPR certification required
