Licensed Practical Nurse (LPN) –– Port Jefferson Station, NY

 

A healthcare provider is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join its nursing team. This role is responsible for delivering high-quality patient care, administering treatments, and supporting residents' health and well-being in a collaborative care environment.

 

Responsibilities

  • Administer medications and treatments in accordance with physician orders and established nursing standards

  • Monitor patient conditions and report changes to appropriate healthcare providers

  • Record vital signs, observations, and nursing interventions accurately and timely

  • Provide direct patient care and assist with daily clinical needs

  • Educate patients and family members regarding care plans, medications, and treatments

  • Collaborate with physicians, nurse practitioners, and interdisciplinary healthcare teams to coordinate care

  • Supervise and support nursing assistants and other assigned staff

  • Maintain accurate medical records and documentation

  • Promote a safe, clean, and patient-centered environment

  • Ensure compliance with healthcare regulations, policies, and standards of care

 

Qualifications

  • Current Licensed Practical Nurse (LPN) license in the state of practice

  • Strong clinical, communication, and organizational skills

  • Ability to work effectively in a fast-paced healthcare environment

  • Experience in skilled nursing, rehabilitation, long-term care, or similar settings preferred
