Responsibilities:
- Oversees medication distribution as stated on physician orders, ensuring strict adherence to Community medication management documentation and procedure, and demonstrating steadfast dedication to resident safety
- Upon thorough assessment, creates personalized service plans tailored to individual care needs while championing resident independence and dignity
- Cultivates communication between physicians, healthcare providers, and family, fostering an effective exchange of communication pertaining to the physical, mental, and social well-being of our residents
- Contributes toward quality assurance and performance improvement initiatives
- Engages in continuous learning and development opportunities through attendance and participation in educational seminars, staff meetings, and other learning experience
Qualifications:
- Current state licensure as Licensed Practical Nurse (LPN)
- Current CPR certification
- Exceptional communication and interpersonal skills
- Must demonstrate the ability to serve with love, compassion, and dignity
