Description

A residential care organization is seeking a Licensed Practical Nurse in Brooklyn, NY to support clinical care for individuals with developmental disabilities in a residential facility. This role is focused on coordinating nursing services, supporting the registered nurse, and providing hands-on care in a team-based setting.

About the Opportunity:

  • Shift: Day shift

  • Schedule: Tuesday to Saturday or Sunday to Thursday

  • Hours: 8am to 4pm or 10am to 6pm

  • Setting: Residential Facility

Responsibilities:

  • Assist the registered nurse in coordinating and following through on healthcare services for the assigned residence.
  • Provide direct nursing care, health instruction, and first aid to individuals receiving services.
  • Administer medications, monitor medication storage, and assist with medication ordering and records.
  • Maintain accurate progress notes, appointment records, and clinical documentation for client reviews and meetings.
  • Support medical appointments, physician consultations, team meetings, infection control practices, and ongoing client care needs.

Qualifications:

  • At least 1 year of experience as a Licensed Practical Nurse in community-based health services for individuals with developmental disabilities.
  • Graduate of an accredited Practical Nursing program
  • Current Licensed Practical Nurse license and registration in the State of New York
  • Current CPR certification.
  • Ability to read, write, speak, and understand English.
  • Strong oral and written communication, organizational, and interpersonal skills.

Desired Skills:

  • Experience with tracheostomy and gastrostomy tube care.
  • Familiarity with working in group homes or apartment-based residential programs.
  • Knowledge of adaptive equipment such as wheelchairs and other supportive devices.
  • Experience supporting interdisciplinary teams and medical meeting coordination.
