    Job Description

    The Licensed Practical Nurse (LPN) provides direct nursing care to residents in a skilled nursing facility under the supervision of a Registered Nurse (RN) or Director of Nursing. The LPN is responsible for administering medications and treatments, monitoring resident conditions, and ensuring care is delivered in accordance with physician orders, care plans, and regulatory standards.
     
    Essential Duties and Responsibilities:
    • Administer prescribed medications, treatments, and therapies accurately and safely.
    • Monitor residents’ vital signs, symptoms, and overall condition; report changes to the RN or physician.
    • Assist in developing, implementing, and updating individualized care plans.
    • Perform wound care, catheter care, tube feedings, IV therapy (if certified), and other skilled procedures.
    • Document all assessments, interventions, and resident responses in the electronic medical record (EMR).
    • Supervise and delegate tasks to Certified Nursing Assistants (CNAs).
    • Respond promptly to resident needs, emergencies, and call lights.
    • Ensure compliance with CMS, state regulations, and facility policies.
    • Participate in interdisciplinary team meetings and quality improvement initiatives.
    • Promote infection control and resident safety practices.
     
    Qualifications:
    • Active LPN license in the state of employment.
    • Graduate of an accredited practical nursing program.
    • Current BLS/CPR certification.
    • Experience in long-term care or skilled nursing preferred.
    • Knowledge of geriatric nursing principles and chronic disease management.
    • Ability to work flexible shifts, including weekends and holidays as needed.
