LICENSED PRACTICAL NURSE (LPN) – SKILLED HOME CARE

We’re looking for an LPN with strong clinical skills—not simply someone looking for a traditional caregiver assignment.

Love and Loyalty Home Care is expanding our skilled care department and seeking an experienced Licensed Practical Nurse (LPN) who is confident providing skilled nursing services in the home.

We are especially interested in nurses with backgrounds in home health, skilled nursing, private duty nursing, rehabilitation, hospital care, or other hands-on clinical environments.

WHAT YOU’LL DO

* Provide skilled nursing services within your authorized scope of practice and the established plan of care.

* Perform assigned nursing observations and assessments within applicable scope and agency protocols.

* Monitor patients closely and recognize changes in condition.

* Promptly communicate clinical concerns or changes to the supervising RN and/or CNO.

* Perform authorized nursing treatments according to physician orders and plans of care.

* Support medication management and patient education as appropriate.

* Maintain detailed, accurate, and timely clinical documentation.

* Communicate professionally with patients, families, physicians, and members of the clinical team.

* Assist with clinical intake and ongoing care coordination as assigned.

* Work closely with RN and nursing leadership to provide consistent, high-quality skilled care.

WHO WE’RE LOOKING FOR

Required:

* Active, unencumbered LPN license in the state where services are provided.

* Hands-on clinical nursing experience.

* Strong patient observation and clinical skills.

* Ability to recognize and report changes in patient condition.

* Strong documentation and communication abilities.

* Dependability and the ability to work independently in a home-care environment.

Strongly Preferred:

* 1–2+ years of clinical nursing experience.

* Home health or skilled home care experience.

* Skilled nursing, private duty, rehabilitation, or hospital experience.

* Experience with physician-ordered treatments and plans of care.

* Experience with EMR systems and electronic clinical documentation.
