About the Role

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team. As an LPN at Hudson Springs, you will have the opportunity to work closely with our healthcare team to deliver high-quality care to our patients. Your responsibilities will include:

• Providing direct patient care, including administering medications, wound care, and vital sign monitoring

• Developing and implementing individualized care plans in collaboration with our interdisciplinary team

• Communicating effectively with patients, families, and healthcare professionals to ensure seamless transitions and optimal care outcomes

• Participating in quality improvement initiatives to continuously improve patient care and outcomes

• Maintaining accurate and detailed records of patient care and progress

Requirements

• Current LPN licensure in the State of Ohio

• High school diploma or equivalent required; associate's degree or higher preferred

• Ability to work effectively in a fast-paced environment and prioritize tasks to meet the needs of our patients

• Excellent communication and interpersonal skills
