Licensed Practical Nurse (LPN) - Weekend Incentive Program

Location: Trillium Place

Current Openings:

• Every Friday, Saturday, and Sunday - 8‑hour shifts

• Nights: 10:30 PM-7:00 AM

• Will work every weekend and every‑other‑holiday rotation.

The Licensed Practical Nurse (LPN), Incentive Weekend Program in skilled care is responsible to assist the RN Manager/RN Supervisor with ensuring all nursing procedures are addressed and resident care issues are handled according to our Philosophy of Care. This care will be performed in accordance with TANDEM LIVING Core Values, Mission Statement and regulations.

QUALIFICATIONS:

• High school diploma or equivalent.

• Current PA LPN license to practice as an LPN.

• Hands‑on CPR certification (must be current or obtained upon hire).

• Two (2) to four (4) years of experience and/or training, preferably in a skilled nursing or retirement facility.

• Basic computer skills required; proficiency with Microsoft Office, including Word and Outlook.

RESPONSIBILITIESS INCLUDE:

• Manages clinical care including medications, vital signs, treatments, and dressing changes.

• Monitors medical devices such as indwelling catheters and peripheral IVs.

• Performs resident mobility support including lifting, turning, and transfers using proper techniques and mechanical lifts with two‑staff assistance.

• Observes and reports health changes and responds appropriately to resident needs.

• Provides skin and wound care including monitoring skin integrity and implementing procedures for skin tears and pressure ulcers.

• Documents and supports team operations including charting, physician visit notes, I&O, pharmacy requisitions, and assisting the RN Nurse Manager while serving as a resource for Caregivers.
