We are seeking a full-time LPN who has the heart and skills to work with home bound patients. Scarlet Home Health Care strives for excellence and has one of the lowest hospitalization rates in the areas. The person we are looking for enjoys appreciation for their hard work, makes a difference providing quality care and wants to be part of a work family not just another employee.
Provide care where ever the patient calls home.
Carry out the plan of care as directed by the patient's physician.
Work with the physician for medication needs and changes to the plan of care.
Ability to complete documentation within an EMR System. (DeVero)
Help decrease the need for re-hospitalization by being actively involved in the patient's plan of care to assess any changes that may be needed.
When appropriate, teach and counsel family members about care and process for disease/injuries.
