    Description

    We are currently searching for a full-time Licensed Practical Nurse (LPN) to join our team! This role will provide compassionate, high-quality nursing care to residents in accordance with established nursing standards and procedures. This position works under the direction of a supervising nurse or clinical leader and plays a key role in ensuring the well-being, safety, and dignity of all residents. Multiple shifts available and both part-time and full-time opportunities available.

    Essential Job Functions 

    • Provide safe, effective nursing care following established policies, procedures, and clinical guidelines. 

    • Accurately administer medications and monitor resident responses; document all observations in the medical record. 

    • Ensure all required medications are in-house, properly ordered, and stocked—especially before weekends, holidays, or forecasted weather events. 

    • Maintain accurate and timely medical records to support communication among residents, physicians, and staff. 

    • Contact residents’ physicians as needed to report concerns or obtain orders. 

    • Communicate effectively with residents and families in alignment with each resident’s care plan. 

    • Provide a thorough and accurate shift-change report to the incoming LPN. 

    • Collaborate with physicians, RNs, and other care providers; obtain and document medical orders according to policy. 

    • Review written and digital resources to better understand residents’ health conditions. 

    • Schedule laboratory testing and follow up on results. 

    • Educate residents and families on care procedures as needed. 

    • Communicate courteously, respectfully, and promptly with residents, families, physicians, and leadership. 

    • Respond to questions from residents, families, physicians, and administrators in a timely manner. 

    • Follow OSHA standards to maintain a safe working environment; report unsafe conditions or behaviors promptly. 

    • Report equipment repair needs to maintenance. 

    • Maintain confidentiality of PHI and all resident information in compliance with HIPAA. 

    • Maintain reliable attendance and arrive prepared to perform job duties daily. 

    Minimum Qualifications 

    • High School diploma or GED. 

    • Graduate of an accredited nursing program. 

    • Valid Pennsylvania LPN license (required per 49 Pa. Code § 21.145). 

    • Minimum of one (1) year related nursing experience preferred. 

    • Ability to work varied shifts, including evenings, weekends, and holidays. 

    • Ability to travel in inclement weather when required. 

    • Occasional overtime may be necessary.
