Licensed Practical Nurse (LPN)

Champlain Valley Senior Community is seeking a compassionate, dependable, and clinically skilled Licensed Practical Nurse (LPN) to support the health, safety, and well-being of residents in our Assisted Living, Enhanced Assisted Living, and Memory Care community.

Responsibilities:

• Provide nursing services within the LPN scope of practice and in accordance with residents’ individualized service plans

• Observe residents and promptly report changes in physical, emotional, cognitive, or behavioral condition

• Administer medications and treatments as authorized and ordered

• Accurately complete documentation in the electronic medication, treatment, and resident-record systems

• Monitor medication administration records and help ensure all required documentation is completed before the end of each shift

• Communicate with physicians, pharmacies, hospitals, families, and other healthcare providers as directed

• Assist with admissions, resident evaluations, service-plan updates, and required clinical documentation

• Review physician orders and help ensure that changes are communicated and implemented promptly

• Respond to resident emergencies, incidents, falls, and changes in condition

• Complete required notifications, documentation, and follow-up after incidents

• Provide guidance and support to HHAs, RCAs, medication technicians, and other care-team members

• Help monitor resident-care assignments and ensure services are provided safely and respectfully

• Support infection-control, medication-management, quality-assurance, and safety practices

• Participate in shift reports, staff meetings, required training, and care conferences

• Maintain residents’ privacy, dignity, safety, choices, and independence

• Follow all facility policies and applicable state and federal requirements

Qualifications:

• Current and unrestricted New York State LPN license required

• Current CPR certification required or must be obtained within the facility’s required timeframe

• Previous experience in assisted living, memory care, long-term care, geriatrics, or a related healthcare setting is preferred

• Knowledge of medication administration, clinical documentation, infection control, and resident safety

• Strong assessment, communication, organization, leadership, and problem-solving skills

• Ability to respond calmly and effectively during emergencies

• Comfortable using electronic health records and medication-administration systems

• Ability to safely perform the physical duties of the position

• Must successfully complete all required background checks, health requirements, training, and employment documentation

Schedule:

Full-time, part-time, and per-diem opportunities may be available. Evening, overnight, weekend, and holiday availability may be required based on facility needs.
