Licensed Practical Nurse (LPN/LVN) – Long Term Care (Developmental Disabilities)

Location: Wayland, NY 14572
Duration: 26 Weeks (13–52 Weeks Available | Extension Possible)
Shift: Night Shift (4x10s)
Schedule: Wednesday–Saturday | 10:00 PM – 8:00 AM
Weekly Hours: 40 Hours/Week
Pay Rate: Local $35–$41/hr | Traveler $42–$50/hr

Position Overview

We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN/LVN) to provide direct patient care for individuals in a long-term care setting, including patients with developmental disabilities. The LPN works under the supervision of an RN and supports medication administration, clinical procedures, and daily resident care needs. This is a mandated facility role requiring flexibility with overtime and floating assignments.

Primary Responsibilities

  • Administer medications, injections, and prescribed treatments.

  • Perform tube feedings, IV therapy (as permitted), and wound care.

  • Collect specimens and perform venipuncture (if certified).

  • Provide colostomy and ostomy care.

  • Monitor and document patient conditions and vital signs.

  • Report changes in resident condition to supervising RN.

  • Assist with ADLs including bathing, grooming, feeding, and mobility support.

  • Perform physician-ordered treatments and clinical procedures.

  • Support lifting, repositioning, and safe patient transfers.

  • Maintain accurate and timely clinical documentation.

  • Float to other facilities as required.

  • Ensure compliance with state and facility regulations.

Requirements

  • Active New York State LPN License

  • BLS Certification required

  • LTC, Skilled Nursing, or Developmental Disabilities experience preferred

  • Strong communication and clinical assessment skills

  • Ability to work independently in high-acuity care environments

  • Willingness to work mandated overtime if required

  • Willingness to float between facilities

  • SCR and SEL clearances required prior to start
