Responsibilities:

Elwyn' s Nursing services are provided for those individuals who live either in Elwyn's residential programs, including Media Campus, or in Community Residential Services in Delaware, Chester and Philadelphia counties. The nursing team is led by a Health Care Coordinator who works in conjunction with the resident's Primary Care Physician and an interdisciplinary team to ensure that the residents are living a healthy life. If necessary, 24-hour nursing care can be provided, and decreasing levels of service are available to residents who function more independently. Nursing services at Elwyn include:

  • Medical case management
  • Medication administration
  • Routine and emergency care
  • Nursing care planning and assessments
  • Treatment as ordered by the Primary Care Physician
 
PRIMARY FUNCTION - As a member of interdisciplinary team, the nurse provides normalized, empathetic, therapeutic and preventative nursing care to clients in an assigned interdisciplinary setting at Elwyn within the scope of the Pennsylvania Practical Nurse Law and in accordance with Pennsylvania DPW Department of Health's standards, IOC, and Elwyn's policies and procedures. Performs the following duties which are within your ability, training, license to perform.

IMMEDIATE SUPERVISOR - The Licensed Practical Nurse reports clinically to: the Department of Nursing, Health Care Manager; Administratively to Assigned Residential Unit, Residential Unit Manager.

CONTACTS - Elwyn personnel, Medical/Dental staff, clients, families, agency staff, visitors, and community agencies.

EDUCATION - Licensed to practice as LPN in Pennsylvania.

EXPERIENCE -Minimum of one year clinical experience in an acute care hospital, current CPR, completion of physical assessment course, history of good interpersonal skills, experience in developmental disabilities preferred.

WORKING CONDITIONS - Ability to work in variety of environments and settings, flexible hours. Exposed to body wastes, discharges, odors and noises.

HAZARDS: Occasional exposure to hazards which may cause serious illness or injury (instruments, equipment, body waste, and infectious diseases).
