Role Description The Licensed Practical Nurse (LPN) at Prestige Home Health Services provides direct nursing care to clients in their homes, including monitoring vital signs, administering medications, performing wound care, and assisting with activities of daily living. The LPN documents all care provided, updates care plans, and communicates changes in client status to registered nurses, physicians, and other members of the care team. The role includes educating clients and caregivers about treatment plans, medication management, and health maintenance, while promoting a safe and comfortable home environment. The LPN collaborates with colleagues to coordinate visits, follow agency policies and regulatory requirements, and maintain accurate clinical records. This is a full-time role based in Milwaukee, WI, serving clients in the local area.

Qualifications

• Active and unrestricted Licensed Practical Nurse (LPN) license in the state of Wisconsin.

• Recent clinical experience in home health, long-term care, or a similar patient care setting.

• Strong assessment and clinical skills, including basic wound care, medication administration, and monitoring of chronic conditions.

• Ability to document care accurately, use electronic health records, and follow established care plans and protocols.

• Reliable transportation for local on-site visits and ability to meet scheduling and on-call requirements as needed.
