Responsibilities
Provide direct patient care in accordance with established care plans, policies, and procedures.

Assist with patient assessments and communicate changes in condition to the appropriate clinical team members.
Support physicians, registered nurses, and other care team members in delivering coordinated care.
Maintain accurate and timely medical records and charting.
Educate patients and family members on care instructions, medications, and wellness practices as directed.
Follow infection control, safety, and regulatory standards to promote a safe care environment.
Respond appropriately to patient needs, concerns, and emergencies within the scope of practice.
Contribute to a respectful, patient-centered, and professional workplace culture.
Qualifications
Current practical nursing licensure in the applicable state or jurisdiction.
Experience providing patient care in a clinical, long-term care, home health, hospital, or similar setting preferred.
Strong understanding of nursing principles, patient monitoring, and basic clinical procedures.
Ability to document care accurately and use electronic health record systems effectively.
Excellent communication and interpersonal skills.
Ability to prioritize tasks, remain organized, and respond calmly in changing situations.
Commitment to delivering respectful, compassionate, and patient-focused care.
Current CPR certification or willingness to obtain may be required.
