About the Role:

As a Licensed Practical Nurse, you will be responsible for providing high-quality, patient-centered care to our residents. Your primary responsibilities will include:

• Assessing residents' physical, emotional, and psychological needs

• Developing and implementing individualized care plans

• Administering medications and treatments as prescribed by physicians

• Monitoring residents' conditions and reporting changes to healthcare providers

• Collaborating with interdisciplinary teams to ensure comprehensive care

Responsibilities:

• Provide direct care to residents, including personal care, wound care, and assistance with activities of daily living

• Monitor and maintain accurate records of residents' care

• Participate in quality improvement initiatives to improve patient outcomes

• Develop and maintain relationships with residents, families, and interdisciplinary teams
