Job Summary
The LPN–Medical Services provides nursing support to individuals receiving medication management and those presenting to the Comprehensive Services Center (CSC) for crisis services. This role ensures medications are administered safely, accurately, and in accordance with physician orders while supporting overall health monitoring and patient education.
 
Responsibilities
Direct Services:
  • Provides nursing services to individuals receiving long-term medication management and those accessing urgent care.
  • Assesses health status, medication compliance, infection control, and emotional needs during appointments.
  • Provides education on MHRC services, mental illness, self-care, and community resources.
  • Educates individuals on medications, including purpose, administration, and potential side effects.
  • Obtains and records vital signs.
  • Verifies physician orders prior to medication administration and administers medications in accordance with established policies and procedures.
  • Maintains and organizes supplies related to medication administration, including injections.
  • Manages sample and Patient Assistance Program (PAP) medications, including receiving, storing, and preparing medications as needed.
  • Ensures proper labeling, storage, and organization of all medications and medication areas.
  • Provides medication administration services at satellite locations as needed.
  • Assesses and monitors for risk, trauma symptoms, and signs of abuse, neglect, or abandonment; follows appropriate reporting protocols.
Documentation and Reporting:
  • Maintains accurate, complete, and timely clinical documentation.
  • Documents all medications administered, including injection sites, and completes required progress notes and service logs.
  • Records relevant observations, including side effects, behaviors, hygiene, and significant interactions.
  • Reports concerns or unusual findings promptly to the Psychiatrist, Nurse Practitioner, or leadership staff.
  • Obtains and maintains required consents and releases of information.

  • Transforms brief notes or session conversations into complete, clinically appropriate documentation 
  • Supports accurate, compliant documentation while allowing staff to review and finalize notes 
  • Provides helpful insights from sessions to support care planning and continuity 
  • Accessible on desktop or mobile for flexibility across different work settings 
Qualifications
  • Graduation from an accredited school of Practical Nursing required.
  • Six months of medical/surgical and/or psychiatric nursing experience required.
  • Valid Florida Licensed Practical Nurse (LPN) license in good standing required.
  • Must demonstrate proficiency in MHRC EHR System within three months of employment. Proficiency in Microsoft Office and email are required. 
  • Strong written and verbal communication skills with the ability to complete accurate clinical documentation. 
  • Excellent customer service and interpersonal skills; ability to work effectively with individuals, families, providers, and staff.
Position Details
This position is a Full Time Days position: Monday through Friday, 8:00am to 4:30pm.

Founded in Jacksonville in May 1977, Mental Health Resource Center, Inc. (MHRC) is a Florida not-for-profit corporation that provides a wide range of behavioral health and social services to the community.  Included in MHRC’s service array are 24-hour emergency services; inpatient psychiatric services for children, adolescents and adults; and numerous outpatient services, including medication management, case management, counseling, supported housing, psychosocial rehabilitation, two programs for the homeless, a therapeutic family program, state hospital liaison services, forensic services, in-jail services, and nine Florida Assertive Community Treatment (FACT) programs for adults with severe and persistent mental illness located in Clearwater, Gainesville, Jacksonville, Kissimmee, Naples, Rockledge, Tampa, and Winter Haven. 

Since 1989, MHRC’s programs have been fully accredited by The Joint Commission.  Inpatient services are licensed by the State of Florida, Agency for Health Care Administration.  MHRC is a certified agency of United Way of Northeast Florida.      

MHRC accepts and directly bills most insurance plans, including Medicaid, Medicare and most private insurance plans.  Individuals who do not have health insurance and cannot afford our standard rates may be eligible for a reduced fee, depending on their income and number of household members.  Some MHRC programs are funded in part by the Florida Department of Children and Families (DCF) and its managing entities, LSF Health Systems, Central Florida Behavioral Health Network, and Central Florida Cares Health System; the federal government; and the City of Jacksonville.
