Your Role

• Provide compassionate, high-quality care to residents in our skilled nursing facility

• Administer medications and treatments as prescribed and within your LPN scope of practice

• Collaborate with RNs, CNAs, and other team members to ensure coordinated, person-centered care

• Monitor residents’ conditions and promptly report changes to the charge nurse or Director of Nursing

• Promote restorative care to encourage residents’ independence and well-being

• Maintain accurate and complete documentation of all nursing care provided

• Support and assist fellow nursing staff to maintain a positive, patient-centered environment

• Ensure resident comfort, safety, and dignity while upholding facility standards

Who We’re Looking For

• Current Licensed Practical Nurse (LPN) license in the state of Illinois

• Graduate of an accredited nursing program

• Compassionate and committed to delivering quality, personalized care

• Team-oriented, reliable, and professional
