Licensed Practical Nurse (Part-time/Nights) - IDD Group Home

A New York City-based healthcare company is seeking a Licensed Practical Nurse (LPN) for an ICF group home in Queens. This part-time overnight role supports adults with intellectual and developmental disabilities by providing nursing oversight, medication administration, and direct health monitoring in a residential setting.

About the Opportunity:

• Shift: Overnight

• Schedule: Tuesday, Friday, Saturday, and Sunday

• Hours: 7pm to 7am

• Setting: ICF Group Home

Responsibilities:

• Oversee and participate in daily personal care, hygiene, ADLs, incontinence care, and environmental cleanliness
• Support mealtime supervision and feeding protocols while following nutritional and medical guidelines
• Administer medications and provide ordered treatments, including G-tube feedings, wound care, urinary drainage maintenance, glucose monitoring, and range of motion therapy
• Monitor resident health by obtaining vital signs, tracking weight, and reporting medical findings to the RN Supervisor and other providers as needed
• Document care, communicate concerns with families and clinical staff, maintain infection control, and help ensure a safe, compliant residence

Qualifications:

• Graduate of an accredited Practical Nursing program
• Current NYS Licensed Practical Nurse (LPN)
• AHA CPR certification or equivalent
• Excellent written and verbal communication skills
• Ability to manage multiple priorities and work collaboratively as part of a team
• Ability to communicate tactfully with personnel, people supported, family members, visitors, government agencies, and the general public
• Ability to implement the goals, objectives, policies, and procedures of the residential nursing services department
