Licensed Practical Nurse (Per Diem) - Mobile Wellness Outreach

A New York City nonprofit organization is seeking a Licensed Practical Nurse (LPN) for a promising Per Diem position with their Mobile Wellness Outreach team throughout the Bronx, Brooklyn, and Queens.

About the Opportunity:

• Shift: Day
• Schedule: One weekday per week
• Hours: 9am to 5pm (with a 1-hour unpaid lunch)
• Setting: Field-based

Responsibilities:

• Conduct health and functional assessments for clients in the community
• Evaluate acute and chronic medical concerns and identify care gaps
• Assess medication management needs and support adherence to care plans
• Collaborate with the multidisciplinary team in case conferences, meetings, and planning sessions
• Provide crisis support and document assessments, notes, and related tools in electronic systems

Qualifications:

• At least 2 years of nursing experience
• Completion of a Practical Nursing program
• Current NYS Licensed Practical Nurse (LPN)
• Experience in Community-based, Mental Health, Behavioral Health, or Substance Use settings
• Experience working with complex Medical and Behavioral Health conditions
• Proficiency with Microsoft Office and electronic health records
• Strong interpersonal and teamwork skills

• Experience working with homeless or formerly homeless populations
• Experience with HIV/AIDS care
• Experience in multidisciplinary care settings
