Job Description:

Licensed Practical Nurse (LPN) responsible for providing patient care in a fast-paced outpatient setting, including rooming patients, triage, and clinical support under the direction of a provider. Ensures efficient patient flow while delivering high-quality, patient-centered care.

Responsibilities:

  • Room patients and maintain efficient patient flow for scheduled and walk-in visits
  • Perform triage (in-person and phone) and prioritize patient needs per protocol
  • Obtain and document vital signs, medical history, and clinical information in the EMR
  • Communicate patient concerns and clinical findings to providers
  • Assist providers with exams, procedures, and patient care
  • Administer medications, immunizations, and treatments as ordered
  • Process prescription refills and patient calls/messages
  • Perform or assist with in-office testing and specimen collection
  • Provide patient education and support for care plans and follow-up
  • Maintain accurate, timely documentation and uphold confidentiality standards

Qualifications:

  • Active, unrestricted LPN license in Vermont
  • Current CPR certification
  • Strong clinical assessment and triage skills
  • Excellent communication and interpersonal skills
  • Ability to multitask and thrive in a fast-paced environment
