About the Role: North Point Senior Homecare in Virginia Beach, VA is looking for a compassionate and dedicated Licensed Practical Nurse (LPN) to join our team on a PRN basis. Responsibilities:
Provide direct patient care to senior clients in a home care setting under the supervision of a Registered Nurse or physician
Administer medications, treatments, and injections as prescribed and document accordingly
Monitor and record patient vital signs, health status, and changes in condition
Assist with wound care, catheter care, and other skilled nursing procedures
Communicate effectively with patients, families, and the care team regarding patient progress and needs
Maintain accurate and timely clinical documentation in compliance with state and agency regulations
Educate patients and caregivers on health management, medications, and care plans
Requirements:
Active Licensed Practical Nurse (LPN) license in the state of Virginia
Previous home care, geriatric, or long-term care nursing experience preferred
Current CPR/BLS certification
Strong clinical assessment and critical thinking skills
Excellent communication and interpersonal skills with a patient-centered approach
Reliable transportation and valid driver's license for home visits
Ability to work independently with minimal supervision in a home care environment
