Our client is a 120-bed skilled nursing and rehabilitation facility undergoing a large-scale renovation under new management. With a strong focus on recovery and personalized care, the team supports residents in achieving optimal health in a peaceful, homelike environment.

Qualifications
- Graduate of an approved school of nursing
- Current LPN license in good standing in New York
- Strong communication, customer service, and organizational skills
- Ability to work well under pressure and collaboratively with staff
- Compassionate, team-oriented approach to care
- Genuine interest in supporting ill, injured, disabled, and elderly residents
- COVID-19 vaccination encouraged (not mandated)
Key Responsibilities
- Provide daily support to unit leadership and respond to resident/family needs
- Deliver direct resident care using nursing judgment and prioritization
- Administer medications and treatments; monitor for effects
- Collaborate with care team to update care plans and communicate findings
- Monitor and document vitals, treatments, and resident responses
- Oversee and evaluate CNA assignments and care delivery
