Job Summary
We are seeking a Licensed Practical Nurse to join our team. In this role, you will primarily be creating and managing a plan of care for Medicaid clients.
Responsibilities
- Coordinate with the team to assess, plan, implement, or evaluate patient care plans
- Document any changes in patient conditions and report on changes
- Nurse will perform Quarterly, annually or every six-month visit with patients for routine assessment and routine recertification.
- Maintain accurate records of patient care and discuss observations with nurse manager
- Assist with admissions, discharges, and patient assessments
- Maintain accurate and timely medical records
- Support and educate patients, caregivers and families regarding care plans.
Qualifications
- Currently Licensed Practical Nurse (LPN) in state of employment
- Previous experience as an LPN is preferred
- Excellent interpersonal and client care skills
- Valid driver’s license
- First aid/CPR certification required
- Prior experience in long-term care, assisted living, rehabilitation, home health, or clinical settings
