Job Summary We are seeking a Licensed Practical Nurse to join our team. In this role, you will primarily be creating and managing a plan of care for Medicaid clients.
Responsibilities
Coordinate with the team to assess, plan, implement, or evaluate patient care plans
Document any changes in patient conditions and report on changes
Nurse will perform Quarterly, annually or every six-month visit with patients for routine assessment and routine recertification.
Maintain accurate records of patient care and discuss observations with nurse manager
Assist with admissions, discharges, and patient assessments
Maintain accurate and timely medical records
Support and educate patients, caregivers and families regarding care plans.
Qualifications
Currently Licensed Practical Nurse (LPN) in state of employment
Previous experience as an LPN is preferred
Excellent interpersonal and client care skills
Valid driver’s license
First aid/CPR certification required
Prior experience in long-term care, assisted living, rehabilitation, home health, or clinical settings
