Position Summary

The Licensed Practical Nurse (LPN) provides direct patient care and nursing support services within long-term care and skilled nursing environments. The ideal candidate is committed to delivering compassionate, resident-focused care while supporting patients with medical, behavioral health, and mental health needs. This role promotes a safe, respectful, and recovery-oriented environment for residents and their families.

Key Responsibilities

  • Provide nursing care within the scope of practice established by Pennsylvania LPN licensure requirements.
  • Monitor, assess, and document resident conditions, observations, and responses to treatment.
  • Administer medications, treatments, and nursing interventions in accordance with facility policies and applicable regulations.
  • Collaborate with nurses, providers, therapists, and other interdisciplinary team members to ensure quality patient care.
  • Maintain accurate and timely medical documentation.
  • Protect resident privacy and confidentiality in accordance with HIPAA and facility policies.
  • Build positive, professional relationships with residents, families, visitors, and colleagues.
  • Participate in orientation, continuing education, and training programs as required.
  • Follow all facility, state, and federal regulations, policies, and procedures.
  • Promote a safe, respectful, and inclusive care environment.

Qualifications

  • Graduate of an accredited Practical Nursing program.
  • Current, unrestricted Licensed Practical Nurse (LPN) license in the State of Pennsylvania.
  • Minimum of one (1) year of LPN experience.
  • Current BLS certification (American Heart Association compliant).
  • Successful completion of all pre-employment screening and clearance requirements, incl background check and fingerprinting.
  • Strong communication and interpersonal skills
  • Ability to work effectively as part of a healthcare team
  • Basic proficiency with Microsoft Office applications
  • Ability to accurately collect, document, and communicate patient information
  • Understanding of behavioral health and age-related care
