The primary focus of this position is to provide high quality and individualized patient care in collaboration with our Nursing Team while supervising CNA and RST staff.St. Ann's Community Offers:

Job Description:

• Conduct thorough assessments of each patient's medical status upon admission and throughout the course of treatment
• Provide regular patient status updates
• Update assessment of each patient's medical status upon admission
• Assist in implementation of the patient's treatment plan under direction of Registered Nurse
• Complete required documentation in an accurate and timely mannerCandidate Requirements:
• Valid NYSED LPN Certification- Required
• Minimum of 1 year experience in working with Elders- Preferred
