Position Summary\u202f\u202f\u202f

The License Practical Nurse (LPN) works under the direct supervision of the Registered Nurse and the Director of Nursing to support the delivery of high-quality patient care. This role involves both clinical and administrative responsibilities, including patient outreach via telephone, accurate documentation in the electronic medical record system and administration of vaccinations in accordance with clinical guidelines. The LPN also plays a key supportive role in clinical operations by assisting registered nurses with staff training and contributing to the optimization of clinical workflows.

  • Provides nursing care including administration of medications and injections to patients as directed by Providers, immunizations, referrals to other providers and community health resources, preauthorization's as required for any testing and health instructions to patients following examinations.
  • Utilizing the nursing process, LPN will provide direct and indirect patient care byidentifyingpatientproblemandconcernand reporting toproviderfor further evaluation.
  • LPN will complete patient intake by documenting past and current medical diagnoses, concerns, medications, allergies, smoking history, and other pertinent social history. LPN will obtain and document patient's vital signs, including blood pressure, respiration rate, pulse, pulse oximetry,heightand weight. All information is documentedutilizingan EMR.
  • Instruct andassistpatients to remove shoes to obtainaccurateweight and inspectfeetof diabetic patients.
  • As a part of the uniform requirement, LPN must wearwrist-watchwith second hand to obtainaccuratevitalsignsprovidesafe and quality patient care.
  • Monitors patient flow andassistsin preparation of examination rooms,chairsand preparation of patients for exam. Effectively work with a care team to deliver safe and quality patient care.
  • Administers vaccines according to policy using two identifiers, provides proper education and information sheets to patients/guardians, and accurately documents information in EMR.
  • Entersappropriate patientdata and all the vaccine information in NJIIS Registry right after the vaccine is administered.
  • Enter refrigerator/freezer temperatures appropriately andin a timely mannerin NJIIS as required by VFC (Vaccines for Children).
  • Maintainan accurate, and daily temperature log for refrigerator/freezer thermometer to assure vaccine and medication safety. Update the log monthly, file the log forpreviousmonth and send a copy to the RN by 5thof each month
  • Inspect and check medications/vaccineexpirationdates on a continuous basis, andmaintaintheexpirationmedication log
  • Utilize time managementskills,multi-task in a fast-paced environment. Maintain nursing competency and patient confidentiality
  • Maintains adequate stock of routine medical supplies; -orders supplies through established purchasing procedures monthly and as needed, andmaintainsall necessary logs updated. Sends copies of all logs to the RN monthly
  • Observes patient, records significant conditions and reactions, and notifies Provider of patient's condition and reaction to drugs, treatments, and significant incidents.
  • Reconciles labs within normal limits, reports critical labs and abnormal results to Provider following OHI policy.
  • Complete various forms as requested by a patient and report toproviderfor final review and management.
  • Ensures chart preparation is completed and entersappropriate ordersbased on outstanding Quality Measures for Provider to review daily
  • Inspects Environment of Care for any safety concerns and infection control issues and intervenes asappropriate. Completes monthly EOCreportandreportsfindings to RN according to protocol.
  • Reports all necessary information and/or unusual occurrencesin accordance withestablished policies and procedures andnotifies RNor the Regional Practice Director of unusual occurrences as they develop.
  • Completes incident reportsimmediatelyas per policy in all required situations.
  • Promotes and deliversoptimumsatisfaction to all patients.
  • Attends and completes all mandatory in-services and attends required meetings.
  • Orients andtrainsstaff asobservedby the Registered Nurse.
  • Develops andmaintainseffective communications by promptly notifying providers, RegisteredNurseand other involved clinical team members of change inpatient'scondition, new orders, and other pertinent information topatient'scare information.
  • Assistswith managing staff inboxes/buckets, prior authorization, referrals, and medical clearance
  • Participates in morninghuddlesto coordinate all clinical activities within the department and facility, as well as daily staff assignments. Recordsinformation discussed in huddle and presents it to the RN.
  • Accepts constructive criticism asdemonstratedby positive behavior changes.
  • Adheres to Corporate Compliance policies.
  • Notifies the Registered Nurse and Chief Medical Officer on matters relating to nursing practices.
  • Provides input to the RN in reference todevelopmentof policies and protocols for patient care.
  • Provides leadership in the development of standards of care for the Department, patient/family education and staff education to address the needs of specific populations.
  • Guides the professional growth of others through role modeling, coaching, and/or mentoring.
  • Provides leadership at a unit level for initiatives designed to improve the quality of care or clinical outcomes for populations of patients and growth of others.
  • Participatesin ongoing quality initiatives.
  • Reports to any OHI site(s) as scheduled by Scheduling Coordinator.
  • Adheres to Corporate Compliance policies.
  • Maintains a passing monthly scorecard threshold at 80%.
  • Other duties as assigned.

Education/Experience/Licensure\u202f

  • Graduate of accredited Licensed Practical Nursing program is required.\u202f
  • One to three years nursing experience in medical center or community health care settings.\u202fOB experience preferred.
  • Active State of New Jersey LPN license is required.\u202f
  • Certified Basic Life Support through American Heart Association is required.\u202f\u202f
  • Bilingual English/Spanish is preferred.\u202f
  • Proficiencyin Microsoft Office 365 isrequired.\u202f\u202f\u202f\u202f
  • Flexibility to meet the hours of thepracticeincluding evenings andweekendsis required.\u202f
  • Flexibility to cover shifts at other current and future OHI sites isrequired.\u202f\u202f
  • Musthave your own reliable transportation throughout Ocean and Monmouth Counties as required by OHI.\u202f
