Licensed Practical Nurse

Licensed Practical Nurse - Full-Time LPN - Monday thru Friday 8:00am - 4:00pm working with Adults with Intellectual Disabilities and Autism located in Washington, PA.

Must be licensed to practice as LPN in Pennsylvania and have a valid PA Driver's License.

\n

\n

Minimum three years general nursing experience, intellectual disabilities nursing experience preferred.

\n

\n

Overall Objective of the Job - To monitor the care and treatment of intellectually and developmentally disabled adults; support adults with intellectual and developmental disabilities in their activities of daily living, such as transfers, range of motion exercises, positioning, crisis intervention, medical appointments etc. To assess individuals for acute problems that might require short-term nursing care and develop plans to address those acute problems. To provide staff with the resources necessary to provide for the care of both the acutely and chronically ill.

Essential Functions of the Job - Evaluate the training needs of staff and conduct trainings related to the health needs of the individuals serviced; Prepares records of trainings and certificates of proficiency, if applicable; Dispense information about and/or coordinate attendance at trainings conducted by other community providers; evaluates the training needs of staff; Maintain ongoing communication with sites regarding health issues of individuals and implementation of healthcare policies and procedures; Directly assess or review assessments of individuals who demonstrate or are reported to have any acute health problems; Conduct quarterly physical assessment of each individual; Monitor all nursing care; Review health status of individual through system of record reviews; Complete follow-up with physicians as necessary; Maintain communication with health care providers; Assist in maintenance of health records; Participate as a member of the Interdisciplinary Team; Implement physicians' orders; Any and all other tasks assigned to ensure the safety and well being of the individuals.

Office of Developmental Programs (ODP) Train the Trainer Course must be completed, if not already certified. Additional duties will include Medication Administration Training of new staff according to ODP Regulations, teach and monitor Annual Medication Administration Practicums with staff to ensure retention of training, coordinating with local pharmacy to ensure individual's medications have been ordered and are available for new cycle, verifying any new prescriptions ordered by doctors, checking in medications once received from the pharmacy, reviewing monthly Medication Administration Records (MARs) for errors prior to the start of the new month, monitoring medications for expiration dates, reordering medications as needed, and any other medication related issues that may arise.

Must be able to assist in lifting over 50 pounds which may require bending, squatting, and turning to achieve proper positioning/placement of individuals. Must have the knowledge and the physical capability to participate in applying physical restraints. Must be capable of maneuvering mobility devices. Must be able to speak and write fluent English, as well as see, hear, stand, sit, walk and drive.

PA Criminal History Checks will be performed on all prospective employees who live, or have lived in PA for the last two years according to 55 Pa. Code, Part I, §51.20 and Part VIII, §6400.21.
