We are seeking a compassionate and skilled Licensed Vocational Nurse (LVN) to join our North and South San Diego County team. As a Home Health LVN, you will provide direct patient care under the supervision of a Registered Nurse (RN), following the plan of care, and collaborating with the interdisciplinary team to support positive patient outcomes. This is a full-time position with opportunities to provide both skilled visits. The pay range for LVN is $35-$50 per visit.

Job Responsibilities

• Provides direct nursing care to patients as defined by the California Nurse Practice Act, Federal and State regulations and agency policies.
• Implements and carries out the plan of care developed by the RN.
• Administers prescribed medications and treatments safely and accurately.
• Provides patient and family/caregiver education relevant to diagnosis, medications, and self-care.
• Maintains timely, accurate documentation and clinical notes consistent with the plan of care.
• Observes and promptly reports changes in patient's condition to the RN and/or Clinical Supervisor.
• Participate in care coordination by communicating identified needs for other disciplines (HHA, PT, OT, ST, MSW, Dietitian).
• Promotes and maintains a safe, therapeutic environment for patients in the home.
