Duties

Perform comprehensive patient assessments, including vital signs, physical examinations, and health history documentation using electronic health record (EHR) systems.
Assist physicians and healthcare providers with clinical procedures such as catheterization, IV insertion, medication administration, wound suturing, and specimen collection.
Manage patient care plans by monitoring progress, updating medical records accurately, and communicating effectively with patients and their families.
Administer injections, manage IV infusions, and provide specialized care such as dementia or Alzheimer's support, hospice care, or memory care.
Support triage processes by evaluating patient symptoms, prioritizing urgent cases in emergency or urgent care settings.
Maintain sterile techniques during procedures like catheter care or phlebotomy while adhering to infection control policies.
Document all clinical activities precisely in EMR/EHR systems while ensuring compliance with HIPAA confidentiality standards.
Assist with discharge planning, medication reconciliation, and client education tailored to individual needs.
Collaborate with interdisciplinary teams including case managers, therapists, and social workers to optimize patient outcomes.
Ensure compliance with long-term care regulations and safety protocols in skilled nursing facilities or assisted living environments.
Requirements

Valid licensure as an LPN from an accredited program; current CPR/BLS certification is required.
Proven experience working in diverse healthcare settings such as hospitals (including ICU or trauma centers), nursing homes, assisted living facilities, or outpatient clinics.
Familiarity with EMR/EHR systems such as Epic, Cerner, Athenahealth, or eClinicalWorks; proficiency in ICD coding (ICD-9/10) is preferred.
Demonstrated knowledge of medical terminology, anatomy, physiology, and pharmacology relevant to patient care.
Experience with specialized procedures including venipuncture, catheterization, tube feeding management, and sterile processing techniques.
Ability to handle multiple priorities efficiently while maintaining attention to detail.
Strong communication skills for effective caregiver-patient interactions and team collaboration.
Knowledge of infection control practices and long-term care regulations applicable to various healthcare environments.
Experience working with individuals with disabilities or complex health conditions such as dementia or traumatic injuries is advantageous.
