We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join a Long Term Care facility located on a hospital campus. The ideal candidate will provide high-quality nursing care, administer medications and treatments, assist with admissions and discharges, and collaborate with the healthcare team to ensure optimal patient outcomes.
Perform patient assessments and administer medications and treatments.
Monitor patient conditions and communicate changes to the physician or nurse practitioner.
Assist with admissions, discharges, and activities of daily living (ADLs).
Provide specialized care including gastrostomy tube (G-tube), urinary catheter, tracheostomy, CPAP, nebulizer treatments, and wound VAC management.
Maintain accurate documentation and ensure quality patient care.
Follow facility policies and collaborate with the interdisciplinary healthcare team.

Active LPN License (state-specific, as required).
Previous Long Term Care (LTC) experience preferred.
Experience with medication administration, patient assessments, and specialized nursing procedures.
Must be flexible to work 8-hour varied shifts, including weekends and holidays (no shift preference).
Proof of current flu vaccination required prior to the contract start date (may be completed during employee health clearance).
Strong communication, teamwork, and patient care skills.
