A growing healthcare organization in Metro Detroit is seeking a Licensed Professional Counselor (LPC)<\/b> to join an integrated behavioral health team. This position operates within a collaborative care model that embeds behavioral health services directly into primary care, allowing clinicians to work closely with physicians, care managers, and interdisciplinary teams to deliver comprehensive patient care.

This role offers a structured outpatient setting with no hospital call or inpatient responsibilities<\/b>, enabling providers to focus fully on patient care in a supportive and collaborative clinical environment.

The position includes a hybrid schedule combining in -clinic and telehealth services<\/b> with a manageable patient volume designed to support high -quality behavioral health care.

Conduct comprehensive mental health assessments and evaluations.
<\/p><\/li>
Develop individualized treatment plans tailored to patient needs.
<\/p><\/li>
Provide outpatient counseling and psychotherapy services to adults and/or children.
<\/p><\/li>
Deliver crisis intervention and behavioral stabilization when appropriate.
<\/p><\/li>
Maintain a structured caseload averaging one patient per hour<\/b> to support quality care.
<\/p><\/li><\/ul>
Work collaboratively with Primary Care Providers (PCPs)<\/b> within an integrated behavioral health model.
<\/p><\/li>
Coordinate care with care managers and interdisciplinary healthcare teams.
<\/p><\/li>
Assist patients in accessing community resources and behavioral health support services.
<\/p><\/li><\/ul>
Maintain accurate and timely clinical documentation, including treatment plans and progress notes.
<\/p><\/li>
Monitor patient progress and modify interventions as necessary.
<\/p><\/li>
Participate in discharge planning and case summaries when appropriate.

Master’s Degree in Counseling, Psychology, or a related behavioral health field<\/b>.
<\/p><\/li>
Active Licensed Professional Counselor (LPC) license<\/b> in the State of Michigan.
<\/p><\/li>
Minimum 3 years of experience providing outpatient behavioral health services<\/b>.
<\/p><\/li>
Experience with mental health assessments, treatment planning, psychotherapy, and crisis intervention.
<\/p><\/li><\/ul>
Experience working within integrated or collaborative care models<\/b>.
<\/p><\/li>
Comfortable coordinating care alongside primary care providers and medication -management clinicians.
