Peritoneal Dialysis - Job Description

Sinai Health System (SHS) is one the largest private safety net healthcare systems in Illinois. GENERAL SUMMARY/BASIC PURPOSE OF JOB

Assists patients and their families with personal and environmental difficulties, which predispose illness or interfere with obtaining maximum benefit from medical care. Collaborates with the nurse case manager and the multidisciplinary team to ensure timely progress through the system and transitions of care. Performs core functions of assessment, advocacy, education and information, referral and linkage, coordination, and counseling and crisis intervention for assigned patients.

ESSENTIAL FUNCTIONS AND DUTIES

  • Completes assessment, evaluates information, and develops and implements plans to meet the identified needs, in collaboration with , patient/family, and the interdisciplinary team.(IDT).
  • Through the evaluation of social and psychological data, assists patients/families in understanding and following medical recommendations in order to restore patient to optimum social and health adjustment and ensure satisfaction with care.
  • Provides education, linkage, advocacy, and coordination for community and governmental services and resources for assigned patients. Provides assistance in obtaining services planned to restore patient to optimum social and health adjustment within patient's capacity.
  • Works in close collaboration with IDT in patient evaluation and treatment to further their understanding of significant social and emotional factors impeding patient's ability to cope with dialysis. Refers patients for transplant evaluation as indicated.
  • Provides support and counseling to patients/families experiencing and/or anticipating issues to adjusting to illness, ESRD, family difficulties, catastrophic diagnoses, changes in family and living situations, loss of independence, grief, and bereavement.
  • Provides education on and assistance with advance directives and end of life care planning.
  • Participates in and may coordinate meeting IDT meetings related to CIP. Participates in patient education sessions for potential patients.
  • Interviews patients and patient's significant others to develop patient and family social history. Prepares assessments, develops goals, provides intervention, and documents all in a timely manner.
  • Identifies appropriate community resources, such as family and community agencies, to provide external patient services. Develops and maintains working relationships with community agencies. Assists patients with obtaining and maintaining insurance coverage necessary for dialysis. Completes required CMS forms such as 2728, and participates in CrownWeb activities as indicated by The Renal Network and CMS Scope of work.
  • Consults, assesses, refers, and coordinates interventions in cases of suspected or confirmed child or adult abuse or neglect or other forms of domestic violence and/or sexual abuse.
  • Prepares and maintains appropriate records and reports, such as assessments and progress notes, in a timely manner.
  • Attends and participates in departmental staff meetings, care conferences, and other meetings as required.
  • Identifies potential high risk complications, including those that may result in admission and readmission barriers to successful home therapy, and addresses with the healthcare team.

JOB REQUIREMENTS:

MINIMUM EDUCATION:

A Master's degree in Social Work (MSW) required.

MINIMUM WORK EXPERIENCE:

Social services experience in a healthcare setting required.

  • Dialysis Social Worker, preferably bi-lingual

REQUIRED LICENSES, CERTIFICATES, REGISTRATIONS:

Licensure by the State of Illinois as a Social Worker (LSW) or Clinical Social Worker (LCSW) required.
