Park Nicollet is looking to hire a Licensed Social Worker, (LISW or LICSW), to join our Outpatient Home Care team! Come join us as a Partner for Good and help us make an impact on the care and experience that our patients and their families receive every day.
Position Summary:
Provides direct social work services to patients, extending services to family members in need of identified social worker services, providing them with psychosocial and environmental support. Makes available a full range of treatment modalities with individuals, families, and groups, based on social work assessment and diagnosis. Coordinates social work services with other departments and providers within the Park Nicollet Health Services i.e., hospice, hospital and clinic-based social workers and out-of-system providers. Provides social work services to the patient and associated community. May provide clinical supervision to other Social Workers. Works with physicians, nurse clinicians, other medical/surgical specialists, and other Park Nicollet Health Services employees. Contact with patients and their families.
Work Schedule: 4 days/week, 8am-4:30pm M-F.
Required Qualifications:
Education, Experience or Equivalent Combination:
Master's degree in social work
Licensure/ Registration/ Certification:
Must have Minnesota (MN) Social Work license upon start date:
MN must have one of the following:
Licensed Independent Social Worker (LISW) or
Licensed Independent Clinical Social Worker (LICSW)
Must have vehicle, driver's license and current auto insurance. Must meet all criteria required under Park Nicollet's Automobile Liability Guidelines
Knowledge, Skills, and Abilities:
Requires basic computer/keyboarding skills.
Demonstrates ability to conduct individual and group therapy sessions.
Demonstrates ability to coordinate and facilitate group interactions.
Demonstrates knowledge of age-related growth and development principles necessary to provide appropriate service and assure safety of patients.
Individuals are responsible for the travel arrangements required to meet the expectations of the position.
Preferred Qualifications:

Master's degree in social work
Licensure/ Registration/ Certification:
MN licensure at the LICSW level.
Knowledge, Skills, and Abilities:
Previous Home Care experience
Advance Care Planning experience
Knowledge of word processing, spreadsheet, and presentation software
