Maintain relationships with referring partners through consistent and timely communication <\/span>

Promote the STARS program and the STARS CRDC Resource Centers program offerings by helping clients to connect with programs that address their needs <\/span>

Evaluate patients (existing, new, and prospective) based on their needs, limitations, and desires <\/span>

Address concerns and goals while maintaining constant communication with the patient <\/span>

Experience in Case Management <\/span>
