Job Description
Key Responsibilities
Patient Care
- Provide skilled nursing services as ordered by the physician and outlined in the plan of care
- Monitor patient condition, vital signs, and report changes to the supervising RN
- Perform treatments such as wound care, medication administration, and basic nursing procedures
- Assist patients with disease management and recovery
Care Coordination
- Work under the direction of the RN to implement patient care plans
- Communicate effectively with physicians, nurses, and interdisciplinary team members
- Participate in case conferences and care coordination activities
- Support continuity of care across all services
Documentation & Compliance
- Complete timely and accurate clinical documentation in the agency’s EHR system
- Ensure compliance with Medicare Conditions of Participation (CoPs) and state regulations
- Follow agency policies and procedures regarding patient care and documentation
- Report incidents, changes in condition, and concerns to the RN/DON
Patient & Family Education
- Educate patients and caregivers on medication management, treatments, and safety
- Reinforce teaching provided by the RN and other clinicians
- Promote patient independence and adherence to care plans
Qualifications
- Active Licensed Vocational Nurse (LVN) license in Nevada
- Graduate of an accredited vocational nursing program
- Minimum 1 year of nursing experience (home health preferred)
- Basic knowledge of nursing principles and patient care practices
- Strong communication and organizational skills
- Ability to work independently under RN supervision
Preferred Qualifications
- Previous home health experience
- Experience with wound care and chronic disease management
- Familiarity with home health EMR systems (e.g., Axxess, WellSky)
- Bilingual (English/Spanish) is a plus
Physical Requirements
- Ability to travel to patient homes within assigned areas
- Ability to lift, assist, and reposition patients as needed
- Ability to stand, walk, bend, and perform physical tasks required for patient care
Work Environment
- Field-based role providing care in patients’ homes
- Flexible schedule based on patient assignments
- Independent work with clinical supervision and support
