Responsibilities:
Conduct initial assessments of patient symptoms and health concerns via phone or online communication.
Triangulate patient information to determine the urgency of care and provide appropriate recommendations.
Guide patients on self-care practices and offer education on medical conditions and treatments.
Document patient interactions and maintain accurate records in our health management system.
Liaison with physicians and other healthcare professionals to coordinate patient care effectively.
Collaborate with the healthcare team to ensure optimal patient experience.
Follow up with patients to ensure they are receiving necessary care or to gather additional information.
Stay current on medical protocols and best practices to provide accurate and effective advice.
Requirements:
Current LVN license.
Basic life support (BLS) certification.
Clinical nursing experience, preferably in triage or a similar role.
Strong communication skills, both verbal and written, with an empathetic approach to patient interactions.
Ability to work independently and make informed decisions in a fast-paced environment.
Attention to detail and strong organizational skills.
Proficiency in using electronic health records (EHR).
