Job SummaryWe are seeking a compassionate and reliable
Licensed Vocational Nurse (LVN) to provide high-quality
direct patient care in a
home health setting. The LVN will work under the supervision of a Registered Nurse (RN) to implement individualized plans of care, deliver nursing treatments, and support positive patient outcomes. This role is ideal for nurses who enjoy independent work, collaboration, and patient-centered care.
Key Responsibilities- Provide direct patient care in accordance with the State Nurse Practice Act
- Implement and follow the plan of care as directed by the Registered Nurse
- Accurately document patient care and clinical observations in a timely manner
- Collect assessment data and communicate changes in patient condition to RNs and physicians
- Collaborate with the interdisciplinary care team
- Assist with coordination of home health services by reporting patient needs
Qualifications & Requirements- Active California LVN license
- 1+ years of nursing experience, home health preferre
- Knowledge of and compliance with professional nursing standards
- Strong verbal and written communication skills
- Excellent organization, documentation, and time management abilities
- CPR certification (current and maintained)
