We are seeking a compassionate and dependable Licensed Vocational Nurse (LVN) to join our San Jose team. In this role, you’ll provide skilled nursing care to patients in the comfort of their homes while working closely with our RNs, clinical leadership, physicians, patients, and families.

What You'll Do

• Provide skilled nursing care to patients in their homes under the direction and supervision of a Registered Nurse (RN).
• Follow the established plan of care and perform ordered preventative, therapeutic, rehabilitative, comfort, and pain-management interventions within the LVN scope of practice.
• Monitor patients for changes in condition and promptly communicate significant findings to the RN, physician, and other appropriate members of the care team.
• Perform treatments and procedures such as medication administration, injections, wound care, vital-sign monitoring, and other skilled nursing services as ordered.
• Educate patients and caregivers regarding medications, treatments, disease management, safety, and the plan of care.
• Assess the home environment for factors that may affect patient safety and make appropriate recommendations to the care team.
• Collaborate with patients, families, caregivers, and interdisciplinary team members to coordinate services and promote positive patient outcomes.
• Obtain and document verbal orders from healthcare providers as appropriate and within the LVN scope of practice.
• Maintain timely, accurate, and complete clinical documentation.
• Help patients achieve greater independence, remain safely at home, and reduce the risk of avoidable hospitalization.

We’re looking for an LVN who combines strong clinical skills with compassion and a genuine commitment to patient-centered care. You are comfortable working independently in a home-based environment while staying closely connected to your clinical team.

You understand that home health is about more than completing a visit—it’s about getting to know your patients, recognizing changes in their condition, educating families and caregivers, and helping each patient achieve the best possible outcome.

Qualifications

• Graduate of an accredited nursing program.
• Current California Licensed Vocational Nurse (LVN) license in good standing.
• Current CPR certification.
• At least one year of clinical nursing experience preferred.
• Home health experience is strongly preferred.
• Strong communication, organization, and clinical documentation skills.
• Ability to work independently while collaborating effectively with the interdisciplinary care team.
• Valid driver's license, reliable transportation, and current automobile insurance.
• Ability to travel throughout San Jose and surrounding service areas to provide patient care in the home.
