Position Summary:
We are seeking a compassionate and reliable Home Health Licensed Vocational Nurse (LVN) to provide skilled nursing visits to patients in South Bay and surrounding areas. This role requires strong clinical judgement
South Bay, Los Angeles County, including:
- Torrance.
- Carson.
- Gardena.
- Hawthorne.
- Lawndale.
- Redondo Beach.
- Manhattan Beach.
- Hermosa Beach.
- San Pedro.
- Lomita.
- Harbor City.
Responsibilities:
- Provide skilled nursing visits in patients' homes according to the physician's Plan of Care.
- Perform head-to-toe assessments and monitor changes in patient condition.
- Provide medication management and patient/caregiver education.
- Perform wound care and other skilled nursing procedures within the LVN scope of practice.
- Monitor vital signs, pain, blood glucose, and other clinical parameters as ordered.
- Communicate changes in condition promptly to the supervising RN, physician, and Care Coordinator.
- Complete accurate and timely clinical documentation.
- Follow agency policies, infection-control practices, patient-safety standards, and home health requirements.
- Coordinate effectively with RNs, physicians, Care Coordinators, therapists, caregivers, and other members of the interdisciplinary team.
Qualifications:
- Current and active California LVN license.
- Current CPR/BLS certification.
- Valid driver's license and reliable transportation.
- Current auto insurance.
- Ability and willingness to travel throughout the South Bay service area.
- Home Health experience is preferred.
- Wound care and other home health clinical skills are a plus.
- Strong communication, organization, and documentation skills.
- Dependable, professional, and patient-focused.
