    About the Role

    As a Licensed Vocational Nurse, you will provide direct patient care under the supervision of a Registered Nurse. You’ll follow individualized care plans, administer treatments, and collaborate with the interdisciplinary team to ensure comfort and dignity for every patient.

    Schedule: Fulltime

    Key Responsibilities:

    • Implement nursing care based on comprehensive assessments and care plans
    • Accurately document patient services to reflect the plan of care
    • Educate patients and families on diagnoses and self-care plans
    • Coordinate hospice services with the interdisciplinary team
    • Use equipment and supplies effectively and efficiently
    • Provide pain and symptom management, evaluating patient response
    • Participate in quality improvement and professional development activities

    Qualifications:

    • Valid California LVN license
    • Minimum 2 years of hospice experience
    • Experience with Home Care Home Base (HCHB) preferred
    • Current CPR certification
    • Valid driver’s license and insured, reliable vehicle

    Pay Range: $40.00 – $44.00 per hour
