We're growing and looking for an experienced Licensed Vocational Nurse (LVN) to join our Hospice Care team!
What You'll Do
As a Hospice LVN, you will provide direct patient care under the supervision of a Registered Nurse (RN). You'll follow the plan of care, deliver treatments, and collaborate with the interdisciplinary team to achieve positive patient outcomes.
Key Responsibilities:
Implement nursing care based on comprehensive assessment and plan of care
Document patient services accurately and timely
Educate patients and families on diagnosis and self-care
Coordinate hospice services and report needs to the care team
Manage pain and symptoms; evaluate patient response to medications
Use equipment and supplies effectively
Participate in quality improvement and professional development
What We're Looking For
Active CA LVN License
Minimum 1 year of LVN experience (hospice experience preferred)
Current CPR certification
Valid driver's license and insured, reliable vehicle
Ability to work independently and as part of a team
