Mobile Health Team Inc. is seeking an experienced Licensed Vocational Nurse (LVN) / Licensed Practical Nurse (LPN)- Home Health for a 13-week travel assignment in Pittsfield, MA.

- Hourly Pay: $ 24.00
- Non-taxed Stipend: $ 1204.00
- Weekly Total Pay: $ 1924.00
- Weekend Requirements: None/Holiday coverage if required
- Traveler Policy: CLIENT PREFERS 45 MIN OR LESS COMMUTE FROM TOR OFFICE (165 Tor Ct, Pittsfield, MA 01201)
- Non-Billable Orientation: 8hrs
** Any/All RTO must be included in profile. ALL SCHEDULING REQUESTS MUST BE MADE AT THE TIME OF SUBMISSION. Facility will NOT approve scheduling requests at/after time of offer/at interview.
The Community Health LPN provides and documents nursing care of the sick and injured at home.
- The LPN intervenes to promote, maintain or restore health to prevent illness and to effect rehabilitation
- The LPN collaborates with other healthcare providers, professionals and community representatives to coordinate care at home
- The LPN evaluates the individual’s and families’ response to interventions.
- Ability to function independently under general guidance of the Director and follows the guidelines and protocols and/or policies and protocols specific to each unit
- Duties require independent action and judgment
- Utilizes time management, communication, planning, delegating, and problem solving in providing care.
Key Responsibilities:
- Observe patients, charting and reporting changes in patients' conditions, such as adverse reactions to medication or treatment, and taking any necessary action.
- Measure and record patients' vital signs, such as height, weight, temperature, blood pressure, pulse, or respiration.
- Administer prescribed medications or start intravenous fluids, noting times and amounts on patients' charts.
Compensation:
- Weekly Pay (hourly) + Non-taxed Stipend (if eligible)

Requirements
Minimum Requirements:
- Must have One year acute nursing care, hospital based preferred.
- Must possess and demonstrate critical thinking skills. Must maintain emotional stability in emergency settings.
- Experience in home health is required
- Certified Home Health Required Including Proficiency with OASIS in highly preferred
- Must have Massachusetts LPN license
- Infusion Therapy Experience is preferred
- Demonstrated knowledge of the principles and practices of nursing.
- Knowledge of and ability to apply community health principles and practice.
- Demonstrated ability to perform independently as a clinician with minimal supervision or assistance.
- Experience with self-scheduling of caseload preferred.
- History of flexibility in assuming varied assignments.
- Current Driver License (and insurance) [Required]
- In order to effectively provide home care, they must have a working knowledge of home care reimbursement and regulatory guidelines.
- Traveler candidate must have their own vehicle. This is a firm requirement.
- Traveler perm address must be included in profile
- Ability to exercise independent judgement.
- Demonstrated interpersonal skills.
- Ability to maintain confidential information
- Ability to develop and maintain records and communicate clinical data in verbal and written form
- Willing to be flexible regarding work hours.
- Ability to drive an automobile in inclement weather.
- Must be willing to accept “on call” responsibilities.

- History of dependability in meeting demands of committed work schedule.
- Must have and maintain current Basic Life Support (BLS) certification during employment
- Excellent communication skills. Ability to work independently.

- Strong communication, professionalism, and reliability
