At Leading Edge Health Services, we believe healthcare does not stop when a patient's appointment ends. Through our Remote Patient Monitoring (RPM) and Remote Therapeutic Monitoring (RTM) programs, we help patients manage chronic conditions, stay on track with treatment plans, and receive support from the comfort of home. Our Licensed Vocational Nurses play a vital role in making that possible.
 
 As an LVN on our remote monitoring team, you will review patient-generated health data, engage patients through phone and electronic communication, provide education and coaching, and help identify potential health concerns before they become bigger problems.

You're a Strong Fit if You Have

• Current and unrestricted LVN/LPN license
• Graduation from an accredited vocational nursing program
• One or more years of nursing experience preferred
•  Experience in chronic care management, telehealth, remote monitoring, home health, ambulatory care, or population health preferred
•  Familiarity with EHR systems and clinical documentation
• Knowledge of chronic disease management and HIPAA requirements
• Strong communication and organizational skills
