Summary

Family Circle of Care (FCC) is seeking a Licensed Vocational Nurse (LVN) to provide comprehensive care to assigned patients and serve as direct support to Providers at our Jacksonville Clinic. This role is essential in delivering high-quality, patient-centered care in a fast-paced clinical environment.

Essential Duties and Responsibilities

(Other duties may be assigned)

  • Provides clinical care as directed by RN, PNP, or Physician
  • Performs patient intake and exit processes
  • Administers immunizations and medications as ordered; documents in EMR
  • Obtains and records vital signs, weight, and patient history
  • Interviews patients to determine medical concerns and documents findings
  • Observes patients for reactions to treatments/medications and reports appropriately
  • Reviews provider orders, lab requests, and follow-up care with patients
  • Documents services for billing purposes
  • Collects lab specimens (urine, blood, sputum, etc.)
  • Maintains and sterilizes equipment and supplies
  • Manages inventory, including stocking and ordering supplies
  • Provides nursing telephone triage and responds to patient inquiries
  • Participates in Continuous Performance Improvement (CPI) initiatives
  • Maintains compliance with all applicable regulations (HIPAA, OSHA, CMS, CLIA, HRSA, NCQA-PCMH, JCAHO, etc.)

Patient-Centered Responsibilities

  • Adjusts to varying workloads while maintaining quality of care
  • Participates in team huddles to support communication and coordination
  • Provides ongoing patient education and documents care in EMR
  • Follows up on labs, imaging, and diagnostic testing as directed

Lead Responsibilities

  • Assists Site Manager with daily operations
  • Supports staffing needs and workflow coordination
  • Assists with staff scheduling and coverage
  • Orders vaccines, maintains inventory, and submits required state reports
  • Serves as liaison between front and back office operations

Qualifications

Education & Experience

  • Associate's degree or equivalent from a vocational nursing program
  • Minimum one year of clinical experience preferred

Certifications & Licensure

  • Current LVN license in the State of Texas required
  • Current BLS Healthcare Provider certification (renewed every 2 years)
  • Maintains all required CEUs and licensure compliance

Skills & Experience

  • Experience with EMR systems (EPIC preferred)
  • Knowledge of Microsoft Office (Word, Excel)
  • Familiarity with point-of-care testing equipment

Preferred

  • Bilingual (English/Spanish)
