Responsibilities Include:

• Providing skilled nursing care to hospice patients
• Coordinating with RN Case Managers and interdisciplinary teams
• Monitoring patient comfort and symptom management
• Educating and supporting patients and families
• Maintaining accurate documentation

Qualifications:

• Current Texas LVN License
• Hospice or home health experience preferred
• Compassionate bedside manner
• Strong communication and organizational skills
• Reliable transportation
