QUALIFICATIONS:
- Graduate of a vocational/technical nursing program
- Current LVN/LPN license in the state of practice
- Knowledge of lab procedures, diagnostic testing, and EMR systems
- Strong documentation, communication, and organizational skills
KEY RESPONSIBILITIES:
- Review medical records, identify gaps in care, and schedule preventive services per physician-approved protocols.
- Collaborate with providers, Nurse Navigators, and families to ensure continuity of care and patient support.
- Educate patients using evidence-based guidelines to promote engagement and health compliance.
- Maintain documentation accuracy, confidentiality, and compliance with all clinical and regulatory standards.
