    Licensed Vocational Nurse (LVN)

    QUALIFICATIONS:

    • Graduate of a vocational/technical nursing program
    • Current LVN/LPN license in the state of practice
    • Knowledge of lab procedures, diagnostic testing, and EMR systems
    • Strong documentation, communication, and organizational skills

    KEY RESPONSIBILITIES:

    • Review medical records, identify gaps in care, and schedule preventive services per physician-approved protocols.
    • Collaborate with providers, Nurse Navigators, and families to ensure continuity of care and patient support.
    • Educate patients using evidence-based guidelines to promote engagement and health compliance.
    • Maintain documentation accuracy, confidentiality, and compliance with all clinical and regulatory standards.
