Position Summary


We are seeking a dedicated and reliable Licensed Vocational Nurse (LVN) to join our skilled nursing team. As an LVN in home health, you will provide direct patient care under the supervision of a Registered Nurse and in accordance with the plan of care. Visits may include wound care, medication administration, chronic disease monitoring, and patient/family education.

Responsibilities


  • Perform skilled nursing visits to patients in their homes in accordance with the care plan

  • Accurately document assessments, care provided, and patient response in EMR (Synergy experience preferred)

  • Monitor and report changes in patient conditions to supervising RN and physician

  • Provide wound care, catheter care, injections, and medication management

  • Collaborate with interdisciplinary team to ensure coordinated, patient-centered care

  • Educate patients and caregivers to promote independence and safety

  • Comply with Medicare/Medicaid and state regulations

Qualifications


  • Current LVN license in the State of Nevada (unrestricted)

  • One (1) year of clinical experience (home health preferred)

  • Valid driver’s license, auto insurance, and reliable transportation

  • CPR Certification (BLS)

  • Ability to work independently and manage time effectively

  • Proficient in electronic documentation (EMR experience preferred)
